Before filling out a registration form, check you are in our catchment area.
If it states you are out of the practice area, please see our page on Out of Area Registrations.
You can also find out which GPs you can register with on the NHS website (may appear as Dr SN CLAY and Partners).
Registering with the surgery
To register with the surgery:
- fill out a New patient registration (adult) form online
- fill out a New patient registration (child under 16) form online
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Everyone in the UK has the right to free care from a GP. If you don’t have proof of ID or address, you can still register. Immigration status or nationality don’t matter, reception won’t ask for immigration documents and won’t share your information with the Home Office unless serious crime is involved.
If you are living outside of our practice boundary, you can still be registered as a patient under certain terms and conditions, which can be found in our out of area information letter (PDF).
Please note, it can take up to 7 working days for us to register you on our system.
Named accountable GP
All of our patients have an allocated named GP, which is your registered GP.
Please note that you can still choose to see the GP of your choice.
