The practice has for a long time provided its patients diagnostic facilities (the ability to test for certain medical conditions) beyond that which is expected of the average general practice.
We have now engaged proactively with the CCG to be early implementers of the following processes that have previously been considered as hospital responsibility.
For the diagnosis and the on-going management of blood pressure we have provided two ABPM’s (portable device that enables a 24 hr recording of blood pressure) which will not only improve our accuracy in diagnosis of high blood pressure but in certain circumstances can better help to monitor patients who are already on treatment.
ECG provision with external quality control and internal interpretation service. This is for patients who may have heart related complaints.
Initiating patients with diabetes injectable therapies within the practice in conjunction with our experienced practice nurse. This enables patients who need to make the transition from tablets to injections a process that can be done at the practice.
COPD (Chronic Obstructive Pulmonary Disease) monitoring and diagnosis with practice based spirometers. We have invested in training our nurses and doctors in an enhanced provision of this service.
If you have been advised by the surgery to submit a COPD review, please use our online COPD Assessment form.
This will hopefully further reinforce the high standards the team at the Poplars have provided for a long time, and will reduce the need for long waits at hospitals that have previously been required for some of the above facilities.
